Once booking an Initial Assessment you will receive our client intake form. We kindly ask you to fill it out in advance so we can really make the most of our time together. It will help us understand your goals and challenges upfront. Prior to your session we will have reviewed all information that you were comfortable sharing and talk about how we can take steps to make change.
