Your first appointment. Book any therapists for an intake session
Service Description
Once booking an Initial Assessment you will receive our client intake form. We kindly ask you to fill it out in advance so we can really make the most of our time together. It will help us understand your goals and challenges upfront. Prior to your session we will have reviewed all information that you were comfortable sharing and talk about how we can take steps to make change.
Cancellation Policy
All in-person appointments held at ​692 Euclid Avenue, Toronto, M6G 2T9, Canada. We are in Toronto's Annex community near Spadina and Bloor. To cancel or reschedule, at a minimum please let us know 48 hours in advance. All appointments to be booked directly with care@canadiantherapy.com.
